I-20/ DS-2019 REQUEST FORM

Congratulations!! You have been admitted to Augustana College and now it's time to prepare to receive your Certificate of Eligibility or DS-2019 or Form I-20. To learn more about these forms, please visit Study in the States.

In order to issue a DS-2019 or form I-20, Augustana College must receive from you documents that demonstrate that you have available to you all the financial resources that you need for at least your first full academic year of study.

Ensure your uploaded bank statement reflects your cost listed on your cost payment worksheet.
The documents must be dated and must not be more than 12 months prior to the date of your classes.
